URGENT NOTICE TO ALL MEDICAL FACILITIES
IMPORTANT RECALL NOTICE
June 14, 1999
Dear
Nursing Home Director:
In cooperation with the United States Consumer
Product Safety Commission, (CPSC), Mealane
Corporation, formerly known as Star Sprinkler Corporation based in Philadelphia,
PA, is recalling "Star"
brand D-1, RD-1, RE-1, E-1, and ME-1 fire sprinkler models made between 1961 and
1976. These
fire sprinklers may not operate properly in a fire, thereby placing you, your residents, and your property at
risk. Although the number of
the five models under recall and presently in service across the country is
unknown, these sprinklers have been widely used in nursing homes and other
health care facilities. The
sprinklers are used in "dry-type"
sprinkler systems only. This recall
does not involve any "Star"
product other than the five models mentioned above, and it is distinct from the
recent recall of "Omega"
brand fire sprinklers. However, you
are urged to identify these models and take corrective action.
The Joint Commission on the Accreditation of Health Care Organizations is
actively supporting the recall in conjunction with the CPSC.
You should immediately check your property’s
sprinkler system for the presence of Star D-1, RD-1, RE-1, E-1, and ME-1 fire
sprinklers made between 1961 and 1976. The
enclosed pictures may help you identify them.
You may also view pictures of them on the web at www.star-recall.com
or call 1-800-866-7807 for assistance. Your
local sprinkler contractor or local fire marshal or authority having
jurisdiction may also be able to determine whether your facility contains these
sprinklers.
If you
have any of these sprinkler heads in your facility, please complete the enclosed
Notice of Claim form immediately and return the form, via mail or fax, to our
firm. Mealane Corporation is
offering consumers and property owners free, listed replacement sprinklers and
reimbursement of removal and replacement labor costs when approved in advance.
