SAFETY RECALL
OLD “STAR” BRAND FIRE SPRINKLERS
June 14, 1999
Dear
State Fire Marshal:
In cooperation with the United States Consumer
Product Safety Commission (CPSC), Mealane Corporation, formerly known as Star
Sprinkler Corporation based in Philadelphia, PA, is recalling “Star” brand
D-1, RD-1, RE-1, E-1, and ME-1 fire sprinkler models made between 1961 and 1976.
These fire sprinklers may not operate properly in a fire, thereby
placing residents and property at risk.
The Star D-1, RD-1, RE-1, E-1 and ME-1 produced from
1961 to 1976 have been widely used in nursing homes and other health care
facilities across the country. They
are also found in schools, warehouses, and supermarkets. The sprinklers are used in dry sprinkler systems only.
This recall does not involve any "Star"
product other than the five models mentioned above, and it is not related to the
recent recall of "Omega"
brand fire sprinklers. The Joint
Commission on the Accreditation of Health Care Organizations is actively
supporting the recall in conjunction with the CPSC.
We are utilizing advertising and direct mail notices to inform property owners about the recall. However, in identifying buildings that contain these five models there is no substitute for personal intervention from fire protection leaders such as yourself. We encourage you to spread the word about this recall among your constituents to protect the public safety and to help identify locations within your state that contain Star D-1, RD-1, RE-1, E-1, and ME-1 fire sprinklers made from between 1961 and 1976 that are still in service. Pictures and diagrams of the models are enclosed. You may also view photos of them on the web at www.star-recall.com or call 1-800-866-7807 for assistance.
If property owners do, in fact, still have these sprinklers in service, they should complete the enclosed Notice of Claim form. Forms should be returned, via mail or fax, to the address below. Mealane Corporation is offering consumers and property owners free, listed replacement sprinklers and reimbursement of the removal and replacement labor costs when approved in advance.
